Am I a Good Candidate for a Rhinoplasty?

Concerns about the shape or size of the nose are common reasons for wanting a nose job. The bridge and tip of the nose are often the areas of the nose that patients want to change. Dr. Narasimhan performs rhinoplasty to improve these characteristics:

  • A hump on the bridge (also called a dorsal hump)

Rhinoplasty generally includes reshaping cartilage and bone to produce the results desired. To do this, Dr. Narasimhan uses one of 2 surgical techniques:

  • Open rhinoplasty is the most common technique and involves making a tiny, horizontal incision along the columella, the small strip of skin that separates the nostrils. After making the incision, Dr. Narasimhan lifts the skin covering the nose. This provides a direct view of the nasal structures.
  • Closed rhinoplasty is performed through incisions made inside of the nostrils. Without a direct view of the nasal structures, Dr. Narasimhan reserves this technique for simpler procedures only.

Your Rhinoplasty Surgery

Dr. Narasimhan performs rhinoplasty as an outpatient procedure using local anesthetic with sedation or general anesthetic.

The procedure typically takes 1 to 2 hours to perform. He usually uses the open technique because it provides a clear view of and access to the internal structures of the nose. Once the surgery is complete, the incision is closed with fine sutures, covered with gauze and a small bandage. A splint may be placed over the nose to protect it during the early healing process.

Recovery After Rhinoplasty

Because rhinoplasty is an outpatient surgery, you will need to have an adult friend or family member drive you home afterward. It’s also recommended that someone stays with you during the first 24 hours immediately following the procedure.

Recovery Timeline

While recovery varies from patient to patient, you can expect to achieve the following milestones within this general timeline:

  • Discomfort: Usually involves stuffiness, not pain, and is controlled with either prescribed or over-the-counter medications for about 3 to 4 days.
  • Swelling and bruising: Peak during the first week and diminish quickly during the second week.
  • Splints: Placed within the nose during surgery and are removed in about 7 days.
  • Return to work & school: Ready to return to non-strenuous activities within 10 to 14 days.

Tips for a Successful Recovery

It’s important to follow the postoperative instructions Dr. Narasimhan provides; they are designed to make your recovery as speedy as possible, with minimal discomfort. Here are some helpful tips:

  • Fill prescriptions prior to surgery.
  • Arrange for someone to drive you home after surgery and help you get settled.
  • Make sure you get plenty of rest in the days following surgery.
  • Sleep with your head elevated and apply cool compresses as directed.
  • Do not take over-the-counter anti-inflammatory medications like ibuprofen.
  • Avoid blowing your nose or wearing glasses until cleared by the doctor.
  • Refrain from smoking because it will impact the healing process.

When Will I See My Final Rhinoplasty Results?

Results continue to evolve during the first 3 months. At that point, you should see what is close to the final results, but expect subtle changes to occur for up to a year.

What’s the Difference Between Surgical and Nonsurgical Rhinoplasty?

Open rhinoplasty leaves a scar that is hidden in the crease where the upper lip meets the columella and is typically virtually imperceptible after it heals. After a closed rhinoplasty, the only resulting scar will be inside the nostrils.

We ask patients to refrain from medications (we provide a list) that can cause bruising, such as certain herbal medications, fish oil, vitamin E tablets, etc. Avoiding nose blowing and heavy activity the first 6 weeks after surgery is also important. Dr. Narasimhan will also provide a steroid medication and anti-nausea medications to reduce inflammation and the risk for vomiting and heaving after anesthesia. Taking these medications will minimize your swelling. Lastly, but most importantly, we strongly emphasize the use of cold compresses over the eyes and forehead at all times, for 7 days after the procedure to minimize swelling.

Dr. Narasimhan asks patients to wait 8 weeks before wearing glasses. If absolutely necessary, glasses can be worn over your nasal cast. But wearing glasses or placing any weight on the nasal bridge too early can stimulate some irritation at the nasal bridge area.

Yes, patients interested in other forms of facial rejuvenation, such as eyelid surgery or a neck lift, can combine the procedures. Combining procedures has 2 main advantages: you will save money by avoiding paying the same fees more than once, and you can recover all at once.

It’s best to postpone cosmetic nose surgery until your nose has finished developing. For girls, that usually occurs by about age 15 or 16. Boys typically reach the same point a couple of years later. Nose surgery to correct breathing difficulties is an exception, and Dr. Narasimhan can discuss the specifics with you if that is the main concern.

There is no upper age limit for a nose job — the only boundary there is to make sure you are healthy enough to undergo surgery — but most patients choose to have rhinoplasty in their 20s and 30s.
