If you’ve ever researched Botox®, you’ve probably heard your injector talk about “units.” But what exactly is a unit of Botox, and does the number of units tell you how much medication you are receiving?
Understanding neurotoxin units, dosage, and reconstitution can help you make more informed decisions about your injectable treatments.

A unit is a measurement of dosage.
Many prescription medications are measured by weight, such as grams (g) or milligrams (mg). Botulinum toxin products are different. Their potency is measured in biological units. The number of units describes the amount of active ingredient or how much medication is being administered.
This distinction is important because a unit of one neurotoxin brand is not necessarily equivalent to a unit of another brand.
Botox®, Dysport®, Daxxify®, Xeomin®, and Jeuveau® are all botulinum toxin products, but they are different medications with their own formulations and dosing systems.
Botulinum toxin is supplied as a powder that must be prepared before it can be injected.
Reconstitution is the process of changing a medication from a powder (or solid state) to a liquid state by adding fluid. For neurotoxin medications, a specific volume of sterile saline is added to create a liquid solution suitable for injection. This resulting solution contains a particular concentration of medication.
Reconstitution is sometimes casually referred to as “dilution,” but technically, reconstitution and dilution are not the same thing.
No. The amount of active medication in the vial does not change simply because saline is added for reconstitution.
What does change is the concentration of the solution within the botulinum toxin vial.
During reconstitution, if a smaller volume of fluid is used a more concentrated solution is created, while a larger volume of fluid results in a less concentrated solution.
This is why comparing the amount of liquid in a syringe—or the number of syringe markings—does not necessarily tell you how many units of neurotoxin you are receiving!
When an injector says you are receiving 20 units of Botox, the number refers to the amount of Botox dosage being administered—not the volume of liquid inside the syringe.
This is an important distinction:
Units = dosage Milliliters (mL) = volume
The amount of liquid in the syringe and the number of units being administered are related, but they are not the same measurement.
Many neurotoxins are administered using a small insulin-style syringe. The markings, sometimes called “tic marks,” measure volume.
The number of tic marks that corresponds to one unit of neurotoxin depends on the concentration of the reconstituted medication. Using tic marks allows your injector to visually convert the number of active medication units to the reconstituted solution volume required for your dose. Knowing how much fluid was used during reconstitution is key for your injector to make this conversion and provide you the correct dose!
Therefore, there is no universal rule that says a certain number of syringe markings always equals one unit of Botox.

This is one reason patients should be cautious about comparing syringe markings between different providers or social media videos. Two injectors can use different reconstitution volumes and still administer the same dosage of active botulinum toxin medication.
While FDA approved Botox dosing exists, there is no single Botox dosage that is appropriate for everyone.
The number of units recommended depends on several factors, including:
● The area being treated
● Muscle strength and activity
● Individual facial anatomy
● Degree of movement
● Treatment goals
● Previous neurotoxin treatments
● The specific neurotoxin being used
For example, the amount of neurotoxin needed to treat the forehead can differ from the amount needed for the glabellar lines (“11s”) or crow’s feet.
An experienced injector evaluates your facial anatomy and muscle movement before determining an appropriate dose and treatment plan that best fits your needs.
If you are comparing injectable treatments between providers, it can be tempting to simply compare the price per unit.
However, unit price alone does not tell the entire story.
Before comparing treatments, consider:
● How many units are recommended?
● Which areas are being treated?
● Is the dosage individualized to your anatomy?
● Who is performing the injection?
● What experience and training does the injector have?
● Which neurotoxin product is being used?
A treatment advertised at a lower price per unit isn't necessarily directly comparable to another treatment using a different neurotoxin product or treatment plan.
